How do I apply?

You can request an assessment by using the Staffordshire EHC Hub link. Further information about the Hub can be found on the ‌ Staffordshire EHC Hub flyer 85KB. If it is the first time you have used the Hub, you will need to click on the Information About EHC button to open up a blank online form.

For parents, carers and young people not wishing to make a request via the Hub, there is a word and PDF version of the form to download on the Requesting an education, health and care assessment page.
